Notice what you can see. Trust what you observe, not what you assume.
Parents, guardians and carers answering on behalf of a child. You are describing what you have noticed, never what the child is feeling inside.

What this is
- A short, guided reflection you complete as the adult who cares for the child.
- A way of putting into words what you have seen over the past week, so a conversation becomes easier to start.
- A set of plain next steps, plus a way to ask for a 45 minute conversation with a wellbeing professional.
What this is not
- It is not a screening tool, a score, or a way to tell whether a child has a condition.
- It cannot tell you what is happening inside your child. Observation is not mind-reading.
- It does not replace a paediatrician, a child psychologist, or your school.
There is no score, no percentage and no result that tells you what is wrong. Nothing here can diagnose a condition, a disorder or a child's mental health, and nothing here replaces a conversation with a qualified professional. Treat it as a prompt to notice things in your own words, then decide what to do next.
The reflection
Take a few minutes with yourself
You are answering as an observer. If you are not sure, choose not sure. A child's behaviour is not a score for anyone, least of all the child.
Before you start
6 questions, about three minutes
Parents, guardians and carers answering on behalf of a child. You are describing what you have noticed, never what the child is feeling inside.
- Six short questions, each answerable in one tap. Every question can be skipped.
- A summary written in the words you used, grouped into themes. No numbers, no grade.
- One small next step you can choose to keep or discard.
